Job Description

Overview:

Essential Functions:

 Implements the Individual Service Plan and offers information for Interdisciplinary Team Meetings.

 Ensures the overall health and safety of the clients in the home, out in the community and during transport through safe driving.

 Provides incoming staff with detailed documentation of the days events through shift

exchange sheets and medical logs.

 Assists clients with their personal finances and providing the necessary documentation for income, expenses and /or corporate funds as directed by the Program Manager.

 Assists with residential activities such as maintaining client’s personal belongings,

organizing and selecting appropriate outfits for work and recreation.

 Involve clients in community activities and events to add to life experiences and community living.

 Completes and/or assists with all home management activities, such as cooking, laundry, cleaning, etc.

 Ensure that clients keep up with their personal hygiene and grooming, such as showers, tooth brushing, shaving, hair cuts and fingernail clipping.

 Promotes a safe and caring co-habitat where clients can learn from staff members and other clients alike and grow while enjoying a healthy and productive lifestyle.

 Understand internal and external policies and follow employee rules of conduct and

performance set forth by Beacon - Keystone.

 Participates in the interdisciplinary process by carrying out the goals and services described in the individual service plan(s).

 Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities.

 Participates in regulatory and agency in-service training provided by Beacon - Keystone

including First Aid/CPR, Safe Crisis Management and Medication Administering Training.

 Transport clients to medical appointments as needed; ensuring accurate record keeping from those appointments.

Responsibilities:

Work Environment:

 Community Living Home environment, which also includes community and recreational

activities.

 May experience exposure to bodily fluids and the need to provide personal and/or private care to clients.

 Shifts may vary depending on staffing needs. Beacon - Keystone is a 24 hour/ 7 day a week facility, therefore varied shift hours will be required.

 Overtime may be required depending on staffing needs.

 Weekend and Holiday shifts are scheduled for all employees.

 Work as scheduled during inclement weather.

 

Physical Abilities:

 Must be able to lift and transfer 50 pounds using proper body mechanics 6-12 times hourly

 Must be able to carry 50 pounds for at least 20 feet

 Must be able to climb and descend 20 stairs carrying 20 pounds

 Must be able to bend to retrieve an object at floor level 6-12 times hourly

 Must be able to readily change direction while walking

 Must be able to run a distance of 100 ft. 1-2 times hourly

 Must be able to walk a distance of one mile

 Must be able to crouch at the knees 6-12 times hourly

 Must be able to kneel on both knees

 Must be able to drive for a period of one hour 3-6 times daily

 Must be able to stand for a period of one hour 6-12 times daily

 Must be able to sit for a period of one hour 6-12 times daily

 Must be able to push/pull 10 pounds

 Must be able to grasp an object with at least one hand

 Must have at least 20/40 combined vision with or without corrective lenses

 Must be able to hear a normal spoken voice with or without hearing assistance

 Must be able to stay awake and alert for entire scheduled shift

 Beacon - Keystone will proactively work with an employee requesting a reasonable

accommodation to be able to complete job duties

Qualifications:

 High School Diploma or High School equivalent

 Experience preferred but not necessary

 Must be at least 19 years of age

 Valid Pennsylvania Driver’s License with an acceptable driving record as determined by

Motor Vehicle Report and insurance guidelines

 

Confidentiality:

 The Direct Service Provider has access to the protected health information of people in the agency’s care. This includes residents at their primary place of employment as well as 

residents of other areas where the Direct Service Provider may be temporarily assigned. The Health Insurance Portability and Accountability Act (HIPAA) regulations apply to

confidential information in all areas of Keystone Community Resources, Inc. and Keystone

Independent Living.

 General Professional confidentiality is also expected in this position.

 

Other Skills:

 Read, write and speak the English language in order to perform job duties.

 Follow written and verbal instructions.

 Perform basic Mathematical functions such as addition, subtraction, multiplication and

division.

 Able to utilize Computer for essential job functions such electronic time and record

keeping

 Manage multiple tasks.

 Complete other duties as assigned.

 

All employees are expected to meet licensing guidelines as an employee of Beacon - Keystone.

This includes:

1. Pennsylvania Criminal Record check

2. Pennsylvania Child Abuse history Clearance

3. Federal Background Check utilizing FBI Fingerprint system

b. bi-annually

4. Physical and Mantoux TB test

5. 40 hours of annual training

6. Signing of a Confirmation Statement indicating there are no civil or criminal liability

for abuse of a Developmentally Disabled individual or Act 13 Violations

7. Sign New Jersey Central Registry of Offenders Against Individuals with

Developmental Disabilities Consent form.

8. All employees will fully cooperate with Beacon - Keystone and any licensing

body/agency in any/all inspections or investigations.
